Patient Transition Specialist

  • Assists with the implementation of Apria's managed care strategic alliances (including exclusive or preferred managed care contracts) as they relate to patient transitions, from initial order intake through revenue confirmation, ensuring full compliance with all company standard operating procedures
  • Contacts patients via telephone to persuade patients to switch service to Apria, identifying and convincing the patients of the corresponding benefits of switching to Apria
  • Validates patient demographic information, equipment, insurance and ordering physician, resolving discrepancies as needed
  • Verifies patient insurance eligibility and benefits and ensures this information is accurately reflected in patient's account, obtaining authorizations from insurance when required
  • Reviews existing medical necessity information for accuracy and compliance and creates new or revised RX/CMNs as required
  • Obtains supporting clinical information for medical equipment from appropriate sources needed for order processing or reimbursement
  • Resolves patient/customer complaints by identifying problems and coordinating appropriate corrective action
  • Demonstrates professional etiquette and courtesy when interfacing with internal and external customers
  • Notifies supervisor of significant findings or concerns identified during the processing of transitioning patients
  • Ensures compliance to company policies and procedures for all product lines, including federal, state, and local regulations
  • Performs other related duties as directed by supervisor

Qualifications:
  • Education or experience equivalent to a high school diploma required; college education preferred.
  • Experienced Apria representative with familiarity with ACIS, Apria's offerings, and general branch operations involved in patient setup are strongly preferred
  • Excellent communication skills are required, including excellent verbal communication skills with strong call-center or phone-based experience
  • Demonstrates persuasiveness with patients and physician offices to successfully overcome objections
  • Able to quickly build rapport in the first few minutes of a call, build trust of patients and sell on the benefits of transitioning to Apria
  • Resourceful and persistent in a friendly way
  • Detail oriented to track progress accurately
  • Familiarity with medical terminology strongly preferred
  • Ability to read and interpret regulations and implement as required by the Company
  • General understanding of health and related types of insurance coverage, as well as the managed care industry and its operations preferred
  • General understanding of durable medical equipment industry preferred
  • Computer knowledge (i.e. Microsoft Word, Excel and Outlook) is required
